- 2ARTs: A Platform for Exercise Prescriptions in Cardiac Recovery PatientsPublication . Pereira, Andreia; Martinho, Ricardo; Pinto, Rui; Rijo, Rui; Grilo, CarlosDue to limited access, increasing costs and an ageing population, the global healthcare system faces significant coverage problems that call for innovative approaches. Health professionals are actively seeking alternative methods to provide care to an increasingly needy population, without increasing human effort and associated costs. eHealth platforms, which use technology to provide patient care, are emerging as transformative solutions for addressing these problems. This study is centered on the demand for a Decision Support System (DSS) in cardiology to enable doctors to prescribe individualized care inside Cardiac Rehabilitation Programmes (CRPs). The 2ARTs project’s main objective is to include a cardiac rehabilitation platform with a DSS within the hospital infrastructure. This DSS uses models to classify patients into different groups, delivering crucial information to assist with decisions regarding treatment. Regarding the DSS, Principal Component Analysis (PCA) emerged as a standout technique for dimensionality reduction, due to its interoperability with clustering algorithms and superior evaluation metrics. The most appropriate clustering technique was determined to be the K-means algorithm, which was supported by the experts analysis. In accordance with the goals of the 2ARTs project, this integration of PCA and K-means provides meaningful insights that improve reasoned decision-making.
- Blockchain Based Network for Tuberculosis: A Data Sharing Initiative in BrazilPublication . Bernardi, Filipe; Lima, Vinicius; Pellison, Felipe; Marques, Paulo Mazzoncini de Azevedo; Rijo, Rui; Galliez, Rafael Mello; Kritski, Afrânio; Alves, DomingosData sharing, information exchange, knowledge acquisition and health intelligence are the basis of an efficient and effective evidence-based decisionmaking tool. A decentralized blockchain architecture is a flexible solution that can be adapted to institutional and managerial culture of organizations and services. Blockchain can play a fundamental role in enabling data sharing within a network and, to achieve that, this work defines the high-level resources necessary to apply this technology to Tuberculosis related issues. Thus, relying in open-source tools and in a collaborative development approach, we present a proposal of a blockchain based network, the TB Network, to underpin an initiative of sharing of Tuberculosis scientific, operational and epidemiologic data between several stakeholders across Brazilian cities.
- Building Bridges for “Palliative Care-in-Place”: Development of a mHealth Intervention for Informal Home CarePublication . Laranjeira, Carlos; Dixe, Maria dos Anjos; Martinho, Ricardo; Rijo, Rui; Querido, AnaIn Palliative Care (PC), family and close people are an essential part of provision of care. They assume highly complex tasks for which they are not prepared, with considerable physical, psychological, social and economic impact. Informal Caregivers (ICs) often falter in the final stage of life and develop distress, enhancing emotional burden and complicated grief. The lack of available and accessible in-person counselling resources is often reported by ICs. Online resources can promote early access to help and support for patient-IC dyads in palliative care. The primary aim of this research is to co-design, develop and test the feasibility of the Help2Care-PAL mHealth app that addresses the needs of ICs of palliative patients cared for at home. This Digital Health Intervention (DHI) in palliative care will be used for education, symptom management, communication and decision-making, to enhance Quality of Life (QoL) of patients and ICs, fostering anticipatory grief and the reach and efficiency of services.
- Challenges on the usability of digital platforms for informal caregivers and health professionals: the case study of Help2CarePublication . Ferreira Caroço, João Eduardo; Marques Gomes, Nuno Miguel; Martinho, Ricardo; Rijo, Rui; Peralta, Teresa; Carvalho, Daniel; Querido, Ana; Dixe, Maria dos AnjosThe usability of digital platforms assumes an important role in the support of health business processes. This is particularly relevant when these platforms automate previously manually-executed activities in a certain process, with the purpose of bringing time, cost and quality benefits both to end-users and health professionals. This is the main objective of the Help2Care e-Health digital platform, where informal caregivers can use a mobile app to access digital training materials on how to properly take care of their patients. Health professionals can, in turn, suggest these materials to their caregivers through a web application, communicate directly with them through a messaging system, and even perform remote health assessment and monitoring. However, the informal caregiver population presents specific attributes which, in general, require special attention for usability. Health professionals can also be challenged by such platforms and their inherent complexity. In this paper, we report the results and challenges of the usability studies performed on both mobile and web applications of the Help2Care platform.

- Decision Support System to Diagnosis and Classification of Epilepsy in ChildrenPublication . Rijo, Rui; Silva, Catarina; Pereira, Luis; Gonçalves, Dulce; Agostinho, MargaridaClinical decision support systems play an important role in organizations. They have a tight relation with the information systems. Our goal is to develop a system to support the diagnosis and the classification of epilepsy in children. Around 50 million people in the world have epilepsy. Epilepsy diagnosis can be an extremely complex process, demanding considerable time and effort from physicians and healthcare infrastructures. Exams such as electroencephalograms and magnetic resonances are often used to create a more accurate diagnosis in a short amount of time. After the diagnosis process, physicians classify epilepsy according to the International Classification of Diseases, ninth revision (ICD-9). Physicians need to classify each specific type of epilepsy based on different data, e.g., types of seizures, events and exams' results. The classification process is time consuming and, in some cases, demands for complementary exams. This work presents a text mining approach to support medical decisions relating to epilepsy diagnosis and ICD-9-based classification in children. We put forward a text mining approach using electronically processed medical records, and apply the K-Nearest Neighbor technique as a white-box multiclass classifier approach to classify each instance, mapping it to the corresponding ICD-9-based standard code. Results on real medical records suggest that the proposed framework shows good performance and clear interpretations, albeit the reduced volume of available training data. To overcome this hurdle, in this work we also propose and explore ways of expanding the dataset.
- Decision Support Systems for Tuberculosis: Protocol for a Scoping ReviewPublication . Lima, Vinícius; Bernardi, Filipe; Pellison, Felipe; Rijo, Rui; Kritski, Afrânio; Galliez, Rafael; Alves, DomingosTuberculosis (TB) represents a global challenge in terms of prevention, care and control. Decision support systems (DSS) can supply the necessary knowledge basis to underpin investigators, policy makers and health personnel actions and to provide crucial elements that can help reducing TB burden. Thus, the objectives of this work are to present the protocol to be followed for carrying out a scoping review to identify topics where DSSs are used, to define appropriate categories and to clarify main outcomes and research gaps. As part of the protocol, five electronic bibliographic databases will be searched for articles from 2006 to 2019 and two investigators will independently screen each work using the study inclusion criteria. Data extraction will be performed, and findings will be reported. The results will be used to provide a broad understanding of how DSSs for TB are being used.
- Developing an Enterprise Architecture Proof of Concept in a Portuguese HospitalPublication . Rijo, Rui; Martinho, Ricardo; Ermida, DiogoA Portuguese hospital with 700 beds and assisting 600 000 inhabitants, in its strategic plan, claimed for the urgent need of using new management approaches for dealing with the complexity of their information systems (IS), the highly coupled Information Technology (IT) infrastructures, and the inefficiency in the communication. This paper describes a proof of concept development using the concepts of Enterprise Architecture (EA) to contribute for a solution for these needs. The work focuses on the improvement of the medical appointment and patients' customer service business processes. Based on key points of existing EA frameworks, we started by using a questionnaire to the top management, and defining a common vision for this specific area, followed by the elicitation of the business processes and the supporting IS and IT infrastructures. Finally, we selected the key performance indicators that will help the hospital to monitor the performance of these processes. With this information gathered we offer a tool for top management to communicate with the clinical and IT areas and, together, take decisions with a common mind map and with the possibility to simulate the impact in the organization of those decisions. Another result of this work is the lessons learned in this proof-of-concept that can be applied in other areas and business processes of the hospital.

This study aims to describe the development process of a multi-domain digital cognitive stimulation program for older adults with cognitive decline. The digital program was developed in four phases: preliminary phase (conceptualization), modeling (experts panel), field test (usability tests), and consensus conference. To ensure software suitability the development process followed also software guidelines for mobile and web applications for older adults. The result was the Digi&Mind digital platform made of a mobile application a backoffice web application. The backoffice web application usability test was conducted with 7 healthcare professionals through the Post-Study System Usability Questionnaire System (PSSUQ). The mobile application test was conducted with 4 older adults and 7 professionals through direct observation user testing. Healthcare professionals considered that the system, the information and the interface had quality. Older adults considered the mobile application as appropriate, useful and easy to use, despite of their low digital experience.
- Help2CARE - Towards a digital platform to help to care for dependent persons and caregiversPublication . Gomes, Nuno; Caroço, João; Martinho, Ricardo; Rijo, Rui; Querido, Ana; Dixe, Maria dos AnjosNowadays, the training of caregivers and patients in self-care is performed in a health professional-to-patient/caregiver oral communication during consultation or discharge from the hospital, which leads to a limited knowledge retention rate either by caregivers and patients in self-care. The Help2CARE project aims to reduce the high rate of readmissions of patients in hospitals due to the lack of training of their informal caregivers. Therefore, we propose to train these caregivers/self-care patients through the provision of training materials that can increase the level of care provided. For this purpose, we present in this paper the Help2CARE digital platform, consisting of a web application and a mobile application that communicate with each other, which enables training materials to be available to caregivers (according to their patients’ needs of care) in a mobile application. With the web application, it is possible for a health professional to manage caregivers and materials to be made available to them, in order to enable those caregivers to access them through the mobile application, among other features, which enables health professionals to accomplish a closer monitoring of the caregivers under their guidance.
