- Can behavioural observations made during the SMART assessment detect the potential for later emergence from vegetative state?Publication . Teixeira, Liliana; Gill-Thwaites, Helen; Reynolds, Frances; Duport, SophieThe management of prolonged disorders of consciousness (PDOC) such as vegetative state (VS) is a major clinical challenge. Presently there are no validated prognostic markers for emergence from VS apart from age, aetiology and time spent in VS. This study explores whether or not the behavioural observation component of the SMART assessment can detect the potential for later emergence from VS. For this retrospective study 14 patients were selected who were originally assessed by the SMART as being in VS. Clinical records showed that seven of these patients have since emerged from VS (Group 1) and the other half have remained in VS (Group 2). The number, type and frequency of behaviours observed during the first five and the last five sessions of the SMART assessment were compared between the groups. The results suggest that the patients who emerged exhibited a significantly higher number of different behaviour types than those who remained in VS (p = .045). Finding an accurate predictor of prognosis for emergence from VS would assist in optimising the treatment and cost-effective management of this population in the future. Since this is a small sample the results should be reviewed with some caution but they suggest that the patients who later emerge from VS can be differentiated behaviourally from those who remain in VS.
- Dependent person in self-care: analysis of care needsPublication . Dixe, Maria dos Anjos; Frontini, Roberta; Miguel Lopes de Sousa, Pedro; Peralta, Teresa; Teixeira, Liliana; Querido, AnaBackground: The aim of this study was to assess the selfcare dependency levels of the dependent person at the time of home discharge and its relationship between (1) the degree of dependency of each self-care domain; (2) the previous dependency levels; and (3) the gender of the dependent person. It also aims to assess the relationship between the degree of dependency of each self-care domain, the length of admission, the length of dependency and the age of the dependent person at the time of discharge. Methods: A cross-sectional study was conducted. The sample comprised hospitalised adults and elderly in the medical services of a Portuguese hospital during the months of March, April and May of 2018. The data were collected by an interview conducted at the time of home discharge from the hospital medical ward. Results: The average age of dependent people of the sample is 80.7 years ( 10.1) with the majority being women (51.7%), with no statistical difference in the mean age according to gender (U = 2205.500; p > 0.05). They were hospitalised on average 11.4 days ( 33.2), most of them (44.0%) due to respiratory problems (85% of which were due to pneumonia). There were no statistically significant differences between the length of the hospital stay, the length of dependency and the participants’ gender (U = 2200.500, p > 0.05; U = 1688.000, p > 0.05). Medication intake was the highest dependency domain amongst participants (41.3%), followed by instrumental activities of daily living (40.6%) and bathing (39.9%). Conclusion: The amount of support required may vary according to the domain that the person is dependent. Thus, it is important to use a robust and reliable assessment tool that will be able to assess the degree of dependency on the various domains of self-care.
- Development and psychometric validation of a multidimensional scale on the self-care needs of dependent peoplePublication . Dixe, Maria dos Anjos; Soares, Elsa; Gomes, José Carlos; Pereira, Cidália; Carvalho, Daniel; Teixeira, Liliana; Sousa, Pedro Miguel Lopes; Amado, Sandra Cristina Fernandes; Peralta, Teresa de Jesus de Almeida; Lopes, Paulo; Querido, AnaObjectives: To develop and validate an interprofessional, multidimensional self-care needs scale of dependent people. Methodology: After a literature search, 61 items were identified and divided into nine self-care dimensions. The items were then reviewed by an expert panel (N = 11) to confirm the content validity of the scale. Refined items were pilot-tested in 12 patients, using a 5-point Likert-type scale. A final version was generated and psychometrically tested in 143 dependent men and women aged 37–102 (80.7 ± 10.1) years. Results: The nine multidimensional self-care needs scales were developed on a broad theoretical and empirical basis. The scales showed very good psychometric properties, having adequate characteristics, satisfactory reliability, and good validity. Conclusions: The scales have good psychometric properties, with minor differential item functioning. The scales can be used by health professional and policy makers to measure dependent people’s self-care needs. Nevertheless, it is necessary to perform further psychometric validations using a larger sample. This is an appropriate tool for application in clinical and research settings to assess patients’ self-care needs.
- Disorders of ConsciousnessPublication . Teixeira, Liliana; Blacker, Danielle; Rocha, NunoA disorder of consciousness (DoC) is a state where consciousness has been affected by damage to the brain. DoC range in the form of a hierarchy, including coma, vegetative state and minimally conscious state. The most common way to assess consciousness is to observe their responses to stimulation. However, observing these responses and detecting purposeful behaviours is extremely challenging. Several studies have shown that misdiagnosis is common. It is crucial to optimise the way consciousness assessments are performed. Clinical management of DoC patients, from treatment of pain to end-of-life decisions, depends on behavioural observations. In the present chapter, we review the challenges posed by the assessment of consciousness and the importance of combining clinical assessment with complementary methods of assessment, such as positron emission tomography, functional magnetic resonance imaging and electroencephalography. According to the diagnosis established, the patient will follow different care pathways. Although therapeutic options of DoC are still limited, basic therapies include artificial nutrition and hydration, physical and occupational therapies as well as sensory stimulation. Pharmacologic trials, deep brain stimulation and multisensory stimulations are some of the therapeutic options for DoCs. Recently, it was removed the requirement to obtain legal sanction for every decision to withdraw clinically assisted nutrition and hydration from people in DoCs. This has led to an entire paradigm shift, from a focus on the diagnosis to a focus on the patient's best interest. Although these decisions will spare the courts' involvement, one should never disregard reaching a correct diagnosis for this vulnerable population.

- Perspective of clients and healthcare professionals about occupational therapy intervention in vocational rehabilitation: A descriptive-exploratory study at a professional rehabilitation centre in PortugalPublication . Ribeiro, Jaime; Inácio, Andreia; Teixeira, Liliana; Pereira, Inês; Lopes, Inês; Gonçalves, IolandaVocational rehabilitation (VR) allows individuals to enter, return and/or maintain employment. After literature review, no evidence of VR in individuals with acquired injuries, in Portugal was found. This paper aims to explore and describe the Occupational Therapy (OT) approach in VR and the client’s and the healthcare professionals’ perspectives, at Centro de Reabilitação Profissional de Gaia. A case study research was carried out with individuals aged 18 to 65 years old, with physical and neurological injuries, and receiving OT intervention; Occupational Therapists and other healthcare professionals working with those clients. To collect and analyse the data, a semi structured interview, focus group and subsequent content analysis by Bardin method were done. Participants’ perspectives were that OT promotes autonomy in occupations, assesses and recommends assistive products and performs functional assessments. Comparing results with literature, the clients and health professionals’ perspective of the OT intervention was consistent with the Occupational Therapist skills and intervention in VR.
